How to Request Transportation

Ask Your Case Manager

Speak with your assigned case manager to request transportation assistance.

Schedule in Advance

Please request transportation at least 48 hours before your appointment when possible.

Confirm Pickup

We'll confirm your pickup time and location the day before your appointment.

Get to Your Appointment

We'll ensure you arrive on time and can provide return transportation if needed.

For same-day urgent appointments, please call our main line as early as possible.
